On January 29th, 2021 Proteus Molecular and Clinical Lab discovered that a former employee breached our Company’s privacy and security policies by emailing a data spreadsheet with patient information to his personal email.

The information in the spreadsheet contained protected health information, including patient name, physician name, clinic name, and in a small number of cases, insurance policy numbers, dates of birth and diagnosis codes. The former employee has attested that the information was not shared with any other parties and has since been deleted from his email account and devices.

Given that the employee did not share the information, we do not believe there is any risk to patients. However, out of an abundance of caution and to comply with our legal requirements, we are providing notification of this incident to our patients.
